Larian Studios has added an option for Karlach to have a happy ending upon her return to Avernus. Karlach will discover a blueprint that can fix her engine, allowing her to permanently remove her explosive heart and leave Avernus. So, by subjecting her to the Illithid transformation, you eliminate any chance she had of living freely as her true self.
